Orthopaedic Rehab

Recovery from musculoskeletal injuries, joint pain, post-surgical rehabilitation and persistent movement limitations. Whether you're dealing with a shoulder, knee, hip, back or neck issue, I'll design a structured, progressive programme that rebuilds your strength, restores function, and gets you back to the activities that matter to you.

Strength, power, mobility and sport-specific conditioning to help you train harder, move more efficiently and reduce your risk of injury. I work with athletes across a range of sports, from runners and cyclists to racquet-sport and team-sport players.

Chronic Condition Management

Exercise is one of the most effective interventions for managing chronic conditions, but it needs to be the right exercise, at the right dose, supervised properly. I work with patients managing conditions like diabetes, hypertension, heart disease, arthritis, respiratory conditions and more, prescribing safe, progressive programmes that improve quality of life and long-term health outcomes.

Posture & Alignment

Chronic neck pain, lower back stiffness, rounded shoulders. These often stem from how we hold ourselves during long hours at a desk or on our feet. I assess your postural patterns and movement habits, identify the areas that need attention, and build a programme focused on strengthening the right muscles, improving mobility and creating lasting change in how your body moves and feels day-to-day.

Assess & Understand

Your first session goes beyond the injury itself. Through a comprehensive movement analysis, strength testing and functional screening, we identify not just the site of pain but the underlying contributing factors: compensatory movement patterns, muscular imbalances, joint mobility restrictions and kinetic chain dysfunctions. The goal is to understand the full clinical picture, so we're treating the cause, not just the symptom.

Prescribe & Progress

Every session is tailored to where you are in your recovery or training. Exercises are prescribed to your current capacity, progressed as your body responds, and adapted based on how you're moving on the day. No two sessions look the same, because no two patients are the same.

Measure & Refine

Regular re-assessments every 4 to 6 weeks track objective progress against your initial baseline. We adjust the programme based on data so you can see exactly how far you've come.

Built on a lifetime of movement

I'm Josh Gilson, a Cape Town-based biokineticist with a BSc in Sport and Exercise Science and an Honours BSc in Biokinetics.

I've always been active. Over the years I've played rugby, cricket and water polo, dabbled in powerlifting and rock climbing, and run marathons and trail races. That range of experience across different sports and movement disciplines gives me a real appreciation for what the body can do, and what it needs when things aren't working the way they should.

What I enjoy most about this work is helping people move better and feel stronger in their day-to-day lives. Whether that's getting a patient back to the sport they love, helping someone manage a chronic condition through exercise, or simply giving a person the confidence to move without pain. That's what drives me as a practitioner.

What is the difference between biokinetics and physiotherapy?+
Physiotherapy focuses on the acute injury phase: pain management, manual therapy and early mobilisation. Biokinetics takes over once you're out of pain and ready to rebuild strength, functional capacity, and sport-specific performance. We frequently collaborate with physiotherapists on shared patients.
Do I need a referral to book?+
No. Biokineticists are first-line practitioners. However, a referral from your doctor or specialist can improve medical aid reimbursement. If you're unsure about your specific scheme, feel free to get in touch and I'll advise.
Does medical aid cover biokinetics sessions?+
Most South African medical aid schemes provide biokinetics benefits, typically from medical savings, day-to-day benefits, or dedicated biokinetics allocations under chronic or PMB conditions.
What should I bring to my first appointment?+
Comfortable training clothes, water, and any relevant clinical documentation: scans, reports, referral letters, or post-surgical protocols. If you don't have documentation available, that's fine; we'll work with what you bring.
How long before I see measurable results?+
Most patients notice meaningful improvement within 3 to 6 sessions. Full recovery timelines are condition-dependent. An ACL reconstruction typically requires 6 to 9 months to return to sport, while postural programmes often show significant change within 8 to 12 weeks. I'll set clear milestones at your initial assessment.
I haven't exercised in years. Is biokinetics appropriate for me?+
Absolutely. A significant part of my practice involves helping people who are returning to exercise after long periods of inactivity. Every programme is prescribed to your current capacity and progressed at a pace that's safe and sustainable.
Do you offer online or home-based sessions?+
Initial consultations are always conducted in person to allow for a thorough physical assessment. I also offer home-based sessions, which are available at an additional fee. Feel free to get in touch to discuss what works best for your situation.
